- Documentation justifying why single-organ transplant and/or maximal medical therapy for heart failure are insufficient or contraindicated for this patient.
- Clinical documentation of deterioration despite optimal therapy (progression notes, treatment history).
- Hemodynamic measurements showing right atrial pressure (>15 mm Hg).
