- To submit a CoQ10 test claim that will generate a denial, use CPT code 82542 on the claim.
- A voluntary Advanced Beneficiary Notice (ABN) may be issued; if an ABN is issued, append the GX modifier.
- To receive a Q10 service denial on a claim, submit CPT code 82542 on the claim form.
- For a voluntarily issued Advanced Beneficiary Notice (ABN), append the GX modifier to the claim.
Key Coverage Criteria
- Coenzyme Q10 (serum or other body fluid) testing is not reasonable and necessary for any diagnosis or clinical purpose and is not covered.
- Presumptiveurine drug testing (UDT) to verify compliance with treatment, identify undisclosed drug use or abuse, or evaluate aberrant* behavior, is consideredmedically necessary, beginning at the start of treatment, as part of a monitoring program tailored to the unique needs of individuals who are:
- Receiving treatment for chronic pain with prescription opioid or other potentially abused medications;or
- Undergoing treatment for, or monitoring for relapse of, opioid or other substance use disorder.
- Presumptiveurine drug testing is also consideredmedically necessaryfor the following:
- To assess an individual when clinical evaluation suggests use of non-prescribed medications or illegal substances;or
