- Diagnostic coronary angiography is considered medically necessary in ANY of the following scenarios:
- Diagnostic coronary angiography is medically necessary for newly recognized resting LV systolic dysfunction (ejection fraction ≤ 40%) when non-ischemic etiologies have been excluded in patients at intermediate or high risk of CAD (using ASCVD Pooled Cohort Equations).
