- Physician prescription is required for home hemodialysis and for use of portable/home hemodialysis systems (NxStage, Tablo).

- Hemodialysis or intermittent peritoneal dialysis for renal failure up to 3 times per week.

- Home hemodialysis when prescribed by a physician for members with end-stage renal disease (N18.6).
