Documentation Required
- Medical records must clearly document medical necessity for each test and include the reason for referral and a clear diagnostic impression.
- Data gathered during NCS should be available showing actual numeric results (latency, amplitude, etc.), preferably in tabular format.
- In cases of review, provide a hard copy of waveforms or a complete written report with interpretation upon request.
- Normal findings and abnormalities must document muscles tested, presence and type of spontaneous activity, characteristics of voluntary motor unit potentials, and interpretation.
Key Coverage Criteria
- NC-stat testing (CPT 95905) is allowed when the beneficiary has a high pre-test or prior probability of Carpal Tunnel Syndrome (CTS), limited to one service per arm.
- Use CPT 95870 for sampling muscles other than the paraspinals associated with the extremities that have been tested.
- Coverage for amniotic and placental-derived product injections/applications for musculoskeletal, non-wound indications is determined by Local Coverage Determination L39139; consult LCD L39139 for specific clinical indications and thresholds.
- Cervical fusion and related cervical spine fusion procedures are the only procedures covered under this policy; other joint or non-cervical spinal procedures (e.g., facet, sacroiliitis, epidural) are not covered by this policy.
- Coverage for a cervical fusion procedure is allowed only when the service meets the reasonable and necessary clinical criteria and frequency limitations specified in the applicable draft Local Coverage Determination (LCD) Cervical Fusion.
- Cervical fusion procedures are covered when they meet the reasonable and necessary clinical criteria and frequency limits specified in Local Coverage Determination (LCD) L39788.
