A diagnosis code corresponding to the beneficiary's diagnosis must be entered on each claim.
Medical records must document the beneficiary’s medical condition sufficient to substantiate the necessity, type, quantity, and frequency of items (including duration, clinical course, prognosis, functional limitations, other interventions and results, and past experience with related items).
To satisfy permanence, the treating practitioner must document in the medical record that the impairment will be of long and indefinite duration.

Enteral nutrition is covered under the Prosthetic Device benefit when the service is reasonable and necessary per the related Local Coverage Determination (LCD) and all Medicare statutory and regulatory requirements are met.
Beneficiary meets the test of permanence when the treating practitioner documents the impairment is of long and indefinite duration (permanence satisfied even if improvement is possible in the future).
Enteral nutrition is indicated when medical records show full or partial non-function or disease of the structures that normally permit food to reach the small bowel.

Special nutrient (disease-specific) formulas (e.g., HCPCS B4149, B4153, B4154, B4155, B4157, B4161, B4162) are covered only when medical records document why a standard formula cannot meet the beneficiary’s metabolic needs (including formulas tried and failed).
When a beneficiary is not in a Part A covered stay, enteral nutrition is eligible for coverage under Part B and may be billed to the DME MAC by the SNF or an outside supplier according to applicable rules.
