- Home health physical therapy services are appropriate when based on impairments of body structure/function and demonstrable functional limitations consistent with the related LCD.
- Tetanus immunization is covered when given for a wound or documented direct exposure to Clostridium tetani (injury-related or exposure-related prophylaxis).
- Tetanus immunization is covered when given for an injury or direct exposure to tetanus (i.e., wound management or exposure prophylaxis).
- Home health physical therapy is covered to improve or restore physical functioning, prevent injury/impairment/activity limitations/participation restrictions, or rehabilitate following disease, injury, or loss of a body part when services require the skills of a qualified physical therapist.
