Policy Summary
Hyaluronan intra-articular injections are covered for painful osteoarthritis of the knee after inadequate response to conservative nonpharmacologic measures and simple analgesics, and may be considered for osteoarthritis of the shoulder. Coverage is limited to knee and shoulder injections, requires medical record documentation of history, exam, diagnostics and prior therapy, and dosing/frequency must follow FDA labeling with repeat courses generally not occurring more often than every six months and limited repeat dosing for the shoulder.
