Policy Summary
Cervical traction devices are covered under the DME benefit when determined reasonable and necessary and when the related LCD criteria are met and documented. Specified HCPCS-coded devices require a face-to-face encounter and a Written Order Prior to Delivery (Final Rule 1713), appropriate use of modifiers KX/GA/GZ, and supplier-held documentation; claims lacking these will be denied or rejected.
