High Frequency Chest Wall Oscillation (HFCWO) devices (HCPCS E0483) are covered under the DME benefit when they meet the reasonable and necessary criteria specified in the related Local Coverage Determination and all Medicare statutory/regulatory requirements. Coverage requires documentation including a face-to-face encounter and a Written Order Prior to Delivery (Final Rule 1713/Article A55426), inclusion of an appropriate diagnosis code on the claim, detailed medical record evidence of retained pulmonary secretions and prior treatments, and correct modifier use (KX if LCD criteria met; GA/GZ otherwise). Coding rules prohibit unbundling replacement vest/hose (A7025/A7026) with a complete system (E0483), and suppliers should contact PDAC for coding guidance.
