Policy Summary
Oxygen equipment is payable under Medicare's DME benefit only when reasonable and necessary as documented per the applicable LCD. Payment for equipment is limited to a 36-month rental period with a 5-year reasonable useful lifetime during which no further equipment payment is made (months 37–60); accessories, maintenance, and stationary contents are generally included in the rental, and new rental periods and equipment changes are tightly restricted to specific conditions (e.g., irreparable damage, theft, qualifying break-in-need, or beneficiary election at RUL end).
