Patient Lifts - Policy Article

Patient lifts (except E0625) may be covered under the DME benefit when reasonable and necessary per the related LCD and all Medicare requirements are met; heavy-duty/bariatric models are included under the specified HCPCS codes. Face-to-face encounters and a Written Order Prior to Delivery (Final Rule 1713) are required for specified codes, PDAC coding verification is required for several codes (E0636, E0639, E0640, E1035, E1036), and E0625 and home modifications are noncovered; required modifiers (KX/GA/GZ) must be used as directed.
