TENS devices (E0720, E0730, E0731) are covered under Medicare's DME benefit only when the reasonable and necessary criteria in the related LCD are met, including documented indications for acute post-operative or chronic pain and evidence of effectiveness after trial. Specified items subject to Final Rule 1713 require a face-to-face encounter and Written Order Prior to Delivery or claims will be denied; supplies are included in rental or purchase allowances (use A4595 for supplies) and certain supply/accessory codes are not payable separately. CMNs are not required for dates of service on/after 2023-01-01, and appropriate claim modifiers (KX/GA/GZ) must be used as specified or claims will be rejected.
