Routine removal of corns and calluses of the feet should be billed with CPT codes 11055-11057 and must use the single code that most accurately reflects the number of keratotic lesions debrided; only one of these CPT codes may be reported per date of service. CPT codes 11305-11308 are not appropriate for routine foot care; when exclusions apply include the applicable Q modifier (Q7, Q8, Q9), report foot/toe location modifiers, append -25 for a separately identifiable same-day E/M, and when using ICD-10 codes under the active care requirement report the last-seen date in CMS-1500 line 19.
