Compounded intravitreal bevacizumab is billable but must be submitted using HCPCS J7999 (claims with J3590 will be returned); for Part B include “Intravitreal bevacizumab, [dose] mg” in Item 19/NTE and for Part A use C9257 with units. Key requirements: report CPT 67028 with RT/LT (or modifier 50 for bilateral), enter days/units '1' for one eye or '2' if both eyes treated in the same encounter when billing J7999, document medical necessity for bundled imaging (e.g., CPT 92134 at 4–6 week intervals) if used, and obtain appropriate informed consent.
