Medicare covers polysomnography and sleep studies for diagnosis of sleep disorders (including OSA) and for evaluating therapy response when performed with physician review and a recording duration of at least six hours. Coverage requires that facilities and personnel meet specified accreditation and credentialing standards, the referring physician's order is retained, the laboratory physician reviews the full raw recording, and reports include detailed monitoring parameters, sleep metrics, respiratory and cardiac findings, and behavioral/EEG/EMG observations. Mobile sleep laboratories are excluded and HST/WatchPAT use must follow specified provider, personnel, instruction, and billing requirements.
