Billing and Coding: Wound Care and Debridement - Provided by a Therapist, Physician, NPP, or as Incident-to Services

Medically necessary skilled debridement (CPT 97597, 97598, 97602) performed by physicians, NPPs, or appropriately certified therapists is covered when documentation supports medical necessity and the complexity of required skills. Dressing changes and routine wound reassessments are generally not separately billable; documentation requirements specify objective wound assessments, instruments/technique used, and a physician-signed therapy plan of care when therapists provide services.
