Policy Summary
Hospice documentation that uses percent weight loss to indicate impaired nutritional status must include the absolute weights (with dates and numeric values) used to calculate the percent change; examples include >10% weight loss over 6 months documented by weights (e.g., 110 lb to 95 lb). Unsupported percent-only weight loss entries without absolute weights or other supporting documentation may lead to denial, and providers are encouraged to include absolute weights when responding to ADRs.
