Billing and Coding: Ophthalmology: Extended Ophthalmoscopy and Fundus Photography

Extended ophthalmoscopy (CPT 92201, 92202) is payable only when documented as an extended exam (e.g., includes a drawing or meets extended ophthalmoscopy standards); general ophthalmoscopy and biomicroscopy are part of the exam and not separately payable. Fundus photography codes (CPT 92227, 92228) are covered when ordered by the treating physician (92227 only for evaluation of at-risk asymptomatic patients such as those with diabetes), fundus photography is treated as bilateral on the MPFS (unilateral services reduced; use modifier -52), and CPT 92201/92202 should not be billed when fundus photography is performed.
