Attended electrical stimulation (CPT 97032) is covered when provided with direct 1-on-1 skilled contact and when paired with appropriate therapeutic procedures; unattended/supervised modalities (G0283) are covered as part of a therapy plan of care without constant direct contact. Pelvic floor electrical stimulation (probe-based or electrode-based) is covered for stress/urge urinary incontinence after a documented 4-week failed PME trial and in cognitively intact patients; wound-related electromagnetic and unattended electrical stimulation (G0329, G0281) are covered only for specified chronic stage III/IV or non-healing ulcers after 30 days of conventional care. Documentation requirements include modality type, areas treated, objective strength/pain measures, wound etiology/duration and prior treatments, and medical necessity justification for use beyond specified visit thresholds (typical training ≤2 visits; improvement expected within 12 visits).
