Attended electrical stimulation (CPT 97032) is covered when delivered with direct 1-on-1 contact, including non-implant pelvic floor probes for stress/urge incontinence after a failed 4-week PME trial; unattended/supervised electrical stimulation (G0283) is covered for pain/swelling if improvement is documented. Electromagnetic therapy (G0329) and unattended electrical stimulation for wounds (G0281) are covered for chronic stage III/IV pressure, arterial, diabetic, and venous stasis ulcers that fail to show measurable healing after 30 days of conventional care. Limitations include noncoverage for TENS in chronic low back pain, exclusions for Bell's palsy/MS/stroke without restoration potential, prohibition on certain concurrent billing, and requirements to document type/area of stimulation, functional or wound improvement (typically within 12 visits), and specific training and PME trial documentation.
