Policy Summary
Noridian disallows use of CPT 37241 for treatment of varicose veins of the lower extremities; prior to 2017-01-01 ClariVein procedures should be billed as CPT 36299 and on/after 2017-01-01 as CPT 36473 (first vein) with CPT 36474 for subsequent veins. The sclerosing agent is bundled into the procedure code and not separately reimbursable; ClariVein must be documented in CMS-1500 Item 19 and RT/LT modifiers should be used when billing both extremities. Providers must consult Noridian LCDs L34209/L34010 for complete coverage criteria.
