Policy Summary
Medically necessary skilled debridement (CPT 97597, 97598, 97602) may be provided and billed by physicians, NPPs, or therapists within scope and licensure, with therapists requiring a physician-certified therapy plan of care. Routine dressing changes, topical applications, and routine wound reassessments are bundled or non-covered; detailed documentation (objective wound assessment and debridement technique/instruments) and adherence to incident-to and E/M billing rules are required for separate reimbursement.
