Medicare covers biofeedback training (CPT 90901, 90912, or 90913) for treatment of stress or urge urinary incontinence in cognitively intact patients only when the medical record documents a failed 4‑week physician‑prescribed pelvic muscle exercise trial. Providers must document the exact time the biofeedback modality was attached to support billed units, and modalities/procedures applied concurrently with biofeedback (e.g., therapeutic exercises, NMES) are considered components of biofeedback and must not be billed separately unless performed outside the biofeedback time interval.
