Policy Summary
Dentists should enroll as specialty 19 for oral surgery procedures like extractions and simple biopsies, and as specialty 85 when trained and practicing primary maxillofacial surgery such as major facial trauma repair or jaw reconstruction. Otolaryngologists should enroll as specialty 04. Providers must self-designate their specialty on the CMS-855I or via PECOS and select the most appropriate specialty to ensure correct coding and Medicare claims processing.
