Policy Summary
Medicare covers cardiac rehabilitation (CR) typically as 36 sessions over up to 36 weeks with each billable session lasting >=31 minutes; up to two sessions may be billed in a day if combined time meets minimums. Intensive Cardiac Rehabilitation (ICR) may be provided as up to 72 one-hour sessions over up to 18 weeks; extensions beyond standard session limits require documentation and KX modifier attestation, submission of ICD-10-CM diagnosis codes with each claim, and contractor discretion where specified.
