CPT codes 17311-17315 are used for Mohs micrographic surgery and include the intraoperative pathologic examination performed by the surgeon; these pathology services should not be billed separately when performed by the Mohs surgeon. When specimens are submitted to and examined by a separate pathologist, pathology may be reported by that pathologist; strict rules govern use of modifier -59 to distinguish separate lesions or diagnostic biopsies, CLIA certification must be reported on all claims, and multiple add-on stage codes must be totaled and billed with appropriate units on a single claim line.
