Cancer TYPE ID (92-gene RT-PCR assay) is billed under CPT 81540 primarily for oncology cases such as tumor of unknown origin. Claims must include the DEX Z-Code™ identifier in specified electronic fields (Loop 2300 NTE01/SV202-7 or SV101-7) or as an attachment on paper claims (Form Locator 80 or Box 19), report one unit (Days/Unit = 1), and include an appropriate ICD-10-CM diagnosis; MolDX expects the test to be ordered only by the treating physician.
