Billing and Coding: MolDX: SULT4A1 Genetic Testing

SULT4A1 genetic testing (single-gene) and any test panels that include SULT4A1 are not covered by Medicare and are statutorily excluded services. Claims for SULT4A1 testing should use CPT 81479; ABNs are not required for statutorily excluded services (use GX for voluntary ABN and GY to indicate exclusion), and required claim-level identifiers (DEX Z-Code) must be entered in the specified SV/loop/box fields for Part A and Part B claims.
