Policy Summary
Independent Diagnostic Testing Facilities may perform the diagnostic procedures listed if they meet the code-specific supervising physician specialties and technician qualifications and obtain Contractor authorization for the HCPCS/CPT codes they will bill. Suppliers furnishing the technical component of advanced diagnostic imaging (CT, MRI, PET) must hold accreditation from one of the four CMS-approved organizations (ACR, IAC, RadSite, TJC); CPT codes listed do not guarantee Medicare coverage and all services remain subject to Medicare rules, LCDs, and medical necessity.
