Billing and Coding: Lab: Bladder/Urothelial Tumor Markers

Noridian covers bladder tumor marker FISH testing only when performed with validated assays (UroVysion® is the only FDA‑approved test) for urine specimens in patients with hematuria for initial diagnosis or recurrence surveillance; non‑validated assays are not covered. Tests must be billed by a CLIA‑certified lab using CPT 88120 or 88121 (append TC for technical‑only claims), and physicians must personally perform the professional interpretation (physician billing is prohibited if interpretation is provided by a lab technician) or risk corrective action.
