To bill the Cancer TYPE ID 92-gene assay, use CPT code 81540 and enter '1' in the Days/Unit field; include the appropriate DEX Z‑Code adjacent to the CPT code in the claim narrative using the specified electronic or paper fields (SV101-7/SV202-7, Loop 2400, Box 19, Block 80). Labs should submit identifier info via SV202-7 (preferred), SV101-7, or the NTE field, and must report an appropriate ICD-10-CM code; MolDX expects the test to be ordered only by the treating physician.
