HTTLPR (serotonin transporter) gene testing is statutorily excluded and not covered due to insufficient evidence. To obtain a formal denial submit CPT 81479 and follow claim-entry instructions (DEX Z-Code placement); an ABN is not required but use GX for voluntary ABN and GY to indicate an ABN on file.
