Policy Summary
Noridian considers Peripheral Nerve Field Stimulation (PNFS/PSFS) not medically necessary and not covered for any condition. Providers submitting claims for PNFS/PSFS must bill CPT 64999 for trial and permanent electrode insertions, indicate trial vs permanent and the PNFS/PSFS descriptor in the claim narrative, report 1 unit of service per procedure, and follow specified Part A/Part B claim-field instructions; Class III (QLK) devices should be reported with CPT 64555 per lead, CPT 64590 for the IPG, and ICD-10-CM M62.85.
