Billing and Coding: Peripheral Nerve Stimulation

Peripheral Nerve Field Stimulation (PNFS/PSFS) is not covered for any condition. To bill for denial, providers must submit CPT 64999 (enter 1 UOS) for both trial and permanent electrode insertion and state “PNFS/PSFS” and whether it is a trial or permanent in the claim narrative (Part B: Loop 2400/SV101‑7 or Item 19 for paper; Part A: SV02‑7 or Block 80/UB04); Class III devices with FDA product class QLK should instead be reported with CPT 64555 per lead, CPT 64590 for the implantable pulse generator, and ICD‑10‑CM M62.85.
