Billing and Coding: Frequency of Hemodialysis

Each hemodialysis session for an ESRD patient must be billed on a separate line (DOS on or after 2007-04-01). Routine coverage is for up to three treatments per week (about 13–14 per month) billed as CPT 90999 with no modifier; additional sessions require supporting medical documentation and must be billed with modifier KX for consideration of extra payment, whereas extra sessions without documentation must be billed with modifier CG and will be denied.
