The LCD permits NPWT (including disposable and durable-device delivery) and wound debridement when medically reasonable and necessary for appropriately selected patients, and recognizes wet-to-dry/dry-to-dry dressings as acceptable when clinically indicated. Experimental or investigational wound care methods are not covered; unusually excessive utilization (debridements >8 in 360 days or NPWT >6 in four months) requires meticulous documentation and may be subject to pre- or post-payment review. Photographic and initial pathology documentation are recommended as best practices, coding guidance will be issued separately, and dressing changes alone generally do not require physician-level skills.
