This billing/coding guidance supplements LCD L33445 for removal of benign and malignant skin lesions; use ICD-10 L82.0 only when legible records document the patient complaint or physician physical findings. Append modifier 25 only when the record documents a significant, separately identifiable E/M by the same physician on the same day, and consult the MPFSDB to determine where the global surgical policy applies.
