Supervised exercise therapy for peripheral arterial disease with symptomatic lower-extremity intermittent claudication is covered when supported by appropriate ICD-10-CM diagnosis codes and medical-record documentation. Coverage is limited to an initial 36 sessions or 12 weeks (whichever comes first); beneficiaries may receive up to an additional 36 sessions only with a new physician referral meeting the same requirements and claims for additional sessions must append modifier -KX.
