Billing and Coding: eVox^® System and Other Electroencephalograph Testing for Memory Loss

eVox®, BrainView NeuralScan, VoxNeuro and similar EEG/ERP testing panels must not be billed using individual CPT codes 92650, 92653, 95816, 95930, 95957 or 96146 but instead reported as the unlisted neurology procedure code 95999 (one unit) with the device name entered in the claim Remarks (Part A) or Narrative (Part B). All documentation must be maintained and available on request, be legible with patient identifiers and the treating provider’s signature, and support the ICD‑10 diagnosis and the CPT/HCPCS code billed.
