Policy Summary
For preoperative cardiac radionuclide stress testing in asymptomatic patients deemed at moderate risk, coverage is allowed only when the medical record documents at least one specified at-risk condition: diabetes with complications, peripheral vascular disease, aortic aneurysm, or cerebrovascular disease. Tests performed in this context without such documentation are not covered; providers must document the qualifying diagnosis in the medical record.
