Covered: Mohs Micrographic Surgery CPT® codes (17311–17315) apply only when a single physician both performs the excision and personally provides the histologic evaluation of 100% of the margins; if the surgeon does not personally perform the histologic evaluation, MMS codes are excluded and standard excision codes (e.g., 11600–11646) must be used. Key requirements: the surgeon should obtain any prior biopsy results performed within 60 days rather than repeat the biopsy when reasonable, a same‑day biopsy may be done if needed to plan MMS, and a nonroutine histochemical stain on frozen tissue is reported with 88314 plus modifier 59 for separate payment.
