Percutaneous vertebral augmentation (PVA) is covered when medically necessary for osteoporotic vertebral compression fractures per the related LCD; coverage may also be available for other medically necessary indications, including cancer-related vertebral disease. Billing must comply with NCCI/OPPS edits, intraoperative venography is bundled and not separately payable, and the medical record must document history, physical exam, and pertinent diagnostic testing; refer to the LCD for any limits on treated levels (the total-level exclusion does not apply to multiple myeloma).
