Billing and Coding: MolDX: Melanoma Risk Stratification Molecular Testing

This billing and coding guidance specifies required claim elements for reporting MolDX melanoma risk stratification molecular testing: select the appropriate CPT and ICD-10-CM codes, enter one unit of service, and include the appropriate DEX Z-Code adjacent to the CPT code on specified claim fields for Part A and Part B. For Part B electronic claims use Loop 2400 or SV101-7 (and Box 19 for paper), and for Part A use SV202-7 for 837I or Block 80 for UB-04; do not add extra characters to the DEX Z-Code on SV101-7.
