GIP multiplex NAAT panels (CPT 87505, 87506, 87507) are treated as a single service (UOS=1) and may be billed when medically reasonable and necessary per LCD L38227. Only one panel may be reported per beneficiary per day (across providers), unbundling into component tests is prohibited, and repeat NAAT testing within seven days during the same episode of diarrhea will be denied. All services must be supported by legible medical record documentation that includes patient identifiers, the responsible provider's signature, and correlation between ICD-10-CM and CPT/HCPCS codes.
