Debridement of mycotic nails (onychomycosis) is covered when reasonable and necessary per Novitas LCD L35013, subject to the LCD's utilization guidelines. CPT codes 11720 and 11721 are subject to NCCI edits and are limited to no more than once every 60 days and may be restricted to six sessions per beneficiary in any 12-month period; review the LCD and NCCI guidance for full documentation and coding requirements.
