Policy Summary
Medicare allows coverage of gastrointestinal pathogen multiplex NAAT panels (CPT 87505, 87506, 87507) when reasonable and necessary per LCD L38229; the panel is treated as a single service (UOS=1) and must not be unbundled. Only one multiplex panel may be billed per beneficiary per day, and repeat NAAT testing within seven days during the same episode of diarrhea will be denied. Documentation must be maintained in the medical record, include patient identifiers and clinician signature, support the chosen ICD-10-CM code(s), and be available to the contractor upon request.
