Medicare covers endovenous stenting only for treatment of severely symptomatic venous obstructions that meet the reasonable and necessary criteria in LCD L38231. Claims must include a specific ICD-10-CM diagnosis linked to the procedure code, and the medical record must contain legible signed notes, documentation of conservative therapy when applicable, device details, and pre/post-deployment imaging; services not meeting LCD criteria are not covered and must be billed with the appropriate modifier.
