Hospices must report a principal hospice diagnosis that is valid under ICD-10-CM Coding Guidelines and consistent with LCD L34558. Unspecified codes for debility, failure to thrive, or dementia, and codes that cannot be used or require special sequencing per ICD-10-CM, are not permitted as the principal hospice diagnosis on hospice claims.
