Billing and Coding: Intraoperative Neurophysiological Testing

Intraoperative neurophysiological monitoring (CPT 95940) is reimbursable when it supports the covered indications in LCD L35003 and is performed one-on-one in the operating room; 95940 is reported per 15 minutes of the monitoring professional's physical presence in the OR and may be billed in addition to outside-OR monitoring (G0453) including baseline interpretation and electrode removal. Documentation must be maintained in the medical record with legible patient identifiers and provider signature and must substantiate the ICD-10 and CPT/HCPCS codes billed; do not bill services not covered by the LCD as if they are covered and use appropriate modifiers for non-covered services.
